Lipomas: Common, Benign, and Usually Harmless
A lipoma is a slow-growing, benign tumor composed of mature fat cells that develops just beneath the skin. They are the most common soft tissue tumor in adults, affecting approximately 1 in every 1,000 people. Most lipomas are small (under 5 cm), soft to the touch, and move freely under the skin when pressed. While they are almost always non-cancerous, there are circumstances where surgical removal is recommended. When Should a Lipoma Be Removed
Most lipomas do not require treatment. However, surgical removal should be considered when:
- The lipoma is growing rapidly — while lipomas grow slowly, rapid enlargement warrants investigation to rule out liposarcoma, a rare but serious malignant tumor
- It causes pain or discomfort — lipomas pressing on nerves or located in areas subject to friction (waistline, under bra straps, neck) can become painful
- It restricts movement — lipomas on joints, forearms, or the back may interfere with normal function
- Cosmetic concerns — visible lipomas on the face, neck, or arms may cause self-consciousness
- Diagnosis is uncertain — if a lump cannot be confidently identified as a lipoma through clinical examination and imaging, excision allows for histopathological confirmation
- Size exceeds 5 cm — “giant lipomas” have a slightly higher risk of malignant transformation and are generally removed
Types of Lipomas
Superficial Subcutaneous Lipoma
The most common type, located between the skin and the muscle layer. These are soft, mobile, and easily felt beneath the skin. They most frequently appear on the trunk, shoulders, neck, and armpits.
Deep Intramuscular Lipoma
These grow within or between muscle layers and may be firmer and less mobile than superficial lipomas. They can be more difficult to remove and require careful surgical planning to avoid muscle damage.
Multiple Lipomatosis
Some patients develop multiple lipomas simultaneously—a condition known as familial multiple lipomatosis. While each individual lipoma is benign, the number and distribution can be distressing, and removal of the most troublesome ones may be recommended.
Diagnosis Before Removal

This typically includes a physical examination, ultrasound imaging to confirm the diagnosis and assess depth, and in some cases an MRI for deeper or atypical lesions. A fine-needle aspiration biopsy may be performed if there is any doubt about the nature of the lump.
Surgical Removal Techniques
Excision Surgery
Traditional surgical excision is the most reliable method for complete lipoma removal. The surgeon makes an incision over the lump, carefully dissects the lipoma from surrounding tissue, and removes it intact. The incision is then closed with sutures. For lipomas under 5 cm, this is typically performed under local anesthesia as an outpatient procedure. The specimen is always sent for histopathological examination to confirm benign status.
Minimal Excision Technique
For smaller lipomas, a minimal excision approach uses a much smaller incision (5-10 mm). The surgeon breaks the lipoma into fragments and removes it through the small opening, resulting in a less noticeable scar. This technique works best for well-encapsulated, superficial lipomas.

For larger lipomas or patients concerned about scarring, liposuction can be used to extract the fatty tissue through a small cannula. While this produces minimal scarring, there is a slightly higher recurrence rate compared to complete excision, and it may not be suitable if pathological examination of the intact specimen is needed.
Recovery After Lipoma Removal
Most lipoma removals are day-case procedures. You can expect mild discomfort managed with over-the-counter pain relief, small adhesive strips or dissolvable sutures that do not require removal, and resumption of normal activities within 1-2 days for small excisions, or up to a week for larger removals. Compression dressings may be applied for 24-48 hours to minimize swelling. The surgical scar typically fades significantly over 6-12 months.
