Initial Evaluation of a Breast Lump

When you present with a breast lump, our evaluation includes a clinical breast examination, imaging studies (ultrasound for women under 35, mammography and ultrasound for women over 35), and when indicated, a biopsy to obtain tissue for pathological analysis. The combination of clinical exam, imaging, and biopsy is known as the “triple assessment” and is the gold standard for evaluating breast lumps.

Types of Breast Biopsy

Fine Needle Aspiration (FNA)

A thin needle extracts cells from the lump for microscopic examination. This quick procedure is performed in the clinic under local anesthesia and provides results within 24-48 hours. FNA is useful for distinguishing between solid and cystic lumps and can often rule out cancer, though it may not provide enough tissue for a definitive diagnosis.

Core Needle Biopsy

A larger needle removes small cylinders of tissue (cores) from the lump. This is the preferred method for initial evaluation of suspicious lumps because it preserves the tissue architecture, allowing the pathologist to determine not just whether cancer cells are present but also the type and grade. Performed under local anesthesia with ultrasound guidance, results are typically available within 3-5 working days.

Surgical (Excisional) Biopsy

When needle biopsies are inconclusive or when the entire lump needs to be removed for both diagnosis and treatment, a surgical biopsy removes the complete lump through a small incision. This is performed as a day-case procedure under local or general anesthesia.

When Lumpectomy Is Recommended

A lumpectomy (wide local excision) removes a breast lump along with a margin of surrounding healthy tissue. It is performed when:

  • A biopsy confirms breast cancer and the tumor is small enough to remove with clear margins while preserving the breast
  • A benign lump is large, growing, or causing symptoms
  • A fibroadenoma (a common benign breast tumor) is enlarging or the patient prefers removal
  • There is atypical hyperplasia or other high-risk findings on biopsy

Lumpectomy for breast cancer is almost always followed by radiation therapy to eliminate any remaining microscopic cancer cells. Studies show that lumpectomy plus radiation provides survival rates equivalent to mastectomy for early-stage breast cancer.

The Lumpectomy Procedure

Under general anesthesia, the surgeon makes an incision directly over or near the lump (often along the areolar edge for cosmetic concealment). The lump and a rim of normal tissue are removed, and the specimen is sent to pathology to verify that the margins are clear of abnormal cells. For cancer cases, a sentinel lymph node biopsy may be performed simultaneously through a separate small incision in the armpit. The procedure typically takes 30-60 minutes, and most patients go home the same day.

Recovery After Breast Lump Surgery

Most patients experience mild to moderate discomfort for 2-5 days, managed with acetaminophen and occasional NSAIDs. You may wear a supportive bra day and night for the first week to minimize movement and swelling. Stitches are typically dissolvable. Avoid heavy lifting and strenuous arm movement for 2-3 weeks. Pathology results are usually available within 5-7 working days and are discussed in a follow-up appointment.

Common Benign Breast Lumps

Fibroadenoma

The most common benign breast tumor in women under 30. These smooth, firm, mobile lumps are not cancerous and often do not require removal unless they are growing, large (over 3 cm), or causing anxiety. Some fibroadenomas shrink on their own over time.

Breast Cysts

Fluid-filled sacs that are common in women aged 35-50. They may fluctuate with the menstrual cycle and can be tender. Simple cysts are benign and require no treatment unless symptomatic—aspiration provides immediate relief.

Fibrocystic Changes

Diffuse lumpiness and tenderness related to hormonal fluctuations. These are not a disease but a normal variation in breast tissue. Management involves symptom control with supportive bras, reduced caffeine intake, and pain medication as needed.
